Course Fee Assistance Form

PROCESS TO FOLLOW WHEN ANTICIPATING USING BRIDGE TO HEALING FOR SMEI COURSES

Requirements:

SMEI requires you to register for a course at least 7 days in advance of the course start date to avoid their late registration charge.  Bridge to Healing requires a waiting period to register of 7 to 21 days after the receipt of your required payment to be assured your payment has cleared.  To allow for the above and to allow Bridge to Healing time to process your request, the deadline for submission of your Request for Course Assistance Form and your required payment is 21 days in advance of the start date of the course for Zelle and PayPal payments or 5 weeks in advance of the start date of the course if you will be mailing a check.  No exceptions to these deadline dates will be granted.

Steps to follow:

1. Complete the Request for Course Assistance Form

2.  Send completed form and Money to Bridge to Healing

• Form may be emailed to bryanshallman@gmail.com
• Money can be sent by Zelle using the email address Bridge2Healing@yahoo.com
• Money can be sent by PayPal using the email address Bridge2Healing@yahoo.com

Please send your PayPal payment as money and not for goods and services.  Sending as a payment for goods and services will delay the processing of your payment at least 10 days.  Remember the required 2.5% PayPal Fee

If paying be Zelle or PayPal, payments should be made 7 days prior to registering for the class.  If mailing a check, the suggestion is that the check and form be mailed 21 days in advance of registration.  This allows for any mailing delays and allows for the check to be processed.

3. Register for the class

All registration for classes are to be done through SMEI and not through Bridge to Healing.

Any online registrations that are received by Bridge to Healing from SMEI with the payment method being “via BTH” and the payment and scholarships form not in hand will be denied.

Therapists making lump sum payments should contact Bryan at bryanshallman@gmail.com to confirm balances in their accounts before registering.
